Depending on whether you are a new or returning student, there will be different requirements in providing health forms to Juniata College.

New Students:

• Complete the New Student Health Packet that outlines required vaccinations, intake forms, and consent to care

Returning Students:

• Complete the Broad Top Health and Wellness registration packet - this must be completed prior to providing health services

Student Athletes:

Prior to participating on a Juniata College sports team, student-athletes must provide documentation of an up-to-date athlete-only physical and insurance information to athletics. The online system ATS is used for this information sharing. New student-athletes will need to create a profile, and returning student-athletes will need to login to access and review their ATS profile. This documentation is required for athletic participation.
